Enhancement of DDM error handling logic on LVM Mirror

Repeated temporary errors, such as "T VRSAS_ERR21TD", should be a trigger of detaching DDMs that has temporary errors on LVM mirror.
(For example, if several "T VRSAS_ERR21TD" errors are occurred in 24hours, the DDM should be recognized the same state as "Missing"
and it will be a trigger of detaching DDM from LVM mirror configuration.)

In the current logic on LVM Mirror, "T VRSAS_ERR21TD" is not a trigger of detaching DDMs from LVM mirror,
because "missing" status is required to detach a DDM from a pair of LVM mirror.

However, when the situation that "T VRSAS_ERR21TD" are occurred repeatedly, the client faced the business impact because of application failure
that was caused non-stable I/O of DDM.
(in the client case, over 150 times of "T VRSAS_ERR21TD" were occured in 12 hours repeatedly, but still not have permanent error.
under this situation, the application had many errors because of I/O issue)

In this regards, the client thinks that repeated "T VRSAS_ERR21TD" should be considered as a trigger of detaching a DDM from LVM mirror.

    There are no plans at this time to make LVM mirroring aware of storage adapter specific recovered errors. LVM mirroring is designed to run any supported AIX storage transport (Fibre Channel, SAS, iSCSI, NVMe) and LVM mirroring will remain unaware of any detailed error information specific to any one adapter or storage protocol.

    AIX is providing enhancements in our 2H22 releases of AIX 7.2 TL 5 and AIX 7.3 which provide a new option to ???chpv -v m" to mark PV missing. This can use this option to bring the rootvg disk offline manually when they encounter disk errors.

    Longer term, AIX continues to consider and investigate means of monitoring and detecting slow or degraded I/O conditions which should trigger a failure of the mirror.
