PMR 70988,7TD presents an instance where the LPAR failed due to an LV hang affecting rootvg. The current analysis is that the failure event began with SAS adapter exceptions that were not properly handled, possibly due to APAR IV69522, and so some in-flight I/Os were never acknowledged back to the LVM layer.
An argument can be made that the LVM should have had its own watchdog timing capability to recognize that I/O had not completed, so that the affected submirrors could have been marked failed to maintain system stability. The LVM should have been able to reasonably survive and work around the hardware (and/or software) failure seen in adapter (and/or driver). This is a request for an LVM watchdog facility.
