qsys2.parse_statement function should return table on create index statements

qsys2.parse_statement function should return table on create index statements.

When processing a 'create view' statement through the parse_statement function, the result set correctly returns tables used in the statement.

When processing a 'create index' statement through the parse_statement function, the result set includes columns, but no table information.

While it's not extraordinarily difficult to parse a 'create index' statement, consistent behavior should be the goal.

Use Case:

A single code base can be used can be used to process multiple SQL statements through the parse_statement function and get consistent result.
