Add the option to log the deletion of MTIs.

Recently I discovered that next to a plan being removed from the Plan Cache, there is another reason why an MTI can be deleted. 
There is a connection between a MTIs and a Plans in Plan Cache. so when a Plan is removed/pruned from the Plan Cache the MTI will go also and is deleted. 
I have seen situations where tables over which MTIs were created, are being deleted. In those cases the MTIs over these tables are also be deleted. 
In one particular situation I have seen nearly over 800 MTIs being created and deleted within a time frame of 5 minutes, while the total number of MTIs stayed the same. 

Analyzing this is extremely hard as there is no track of MTIs being deleted. In this particular situation running a Plan Cache Event Monitor does not give you the answers

    IBM does not intend to provide a solution to this Idea at this time, so it is being closed. While this Idea could be implemented, the effort required is not trivial and is unlikely to be prioritized for implementation in the next several years.

    Db2 for i includes technologies that could be used to understand that MTIs are being deleted.
    1. The QAPMSQLPC bucket within Collection Services includes an MTI Deleted count, which gets captured on every CS interval. See this Gist for an example of how to extract this detail: https://gist.github.com/forstie/e4a4d253958e55b5de0801aaf6c56f60
    2. The Index Advisor includes many MTI attributes, including MTI created. While this is not an exact match for MTI deleted, it can be inferred that MTIs are being deleted by recognizing an increasing count of MTIs created.
